Indications
The drug Spirocard 100 is classified as a diuretic (water pill). It is primarily used to treat excessive blood pressure and heart failure. It lessens the edema (swelling) brought on by a few other conditions. Low potassium levels in the body are treated with this medication as well (hypokalemia).
Pharmacology
Spironolactone may be a particular pharmacologic enemy of aldosterone, acting essentially through competitive official of receptors at the aldosterone-dependent sodium-potassium trade location within the distal convoluted renal tubule. Spironolactone causes expanded sums of sodium and water to be excreted, whereas potassium is held. Spironolactone acts both as a diuretic and as an antihypertensive sedate this instrument. It may be given alone or with other diuretic operators which act more proximally within the renal tubule. Aldosterone interatomic with a cytoplasmic mineralocorticoid receptor to improve the expression of the Na+ K+ ATPase and the Na+ channel included in a Na+ K+ transport within the distal tubule. Spironolactone ties to this mineralocorticoid receptor, blocking the activities of aldosterone on quality expression. Aldosterone may be a hormone; its essential work is to hold sodium and discharge potassium within the kidneys.
Dosage & Administration
Edema in adults (congestive heart failure, hepatic cirrhosis, or nephrotic syndrome): An initial daily dosage of 100 mg of Spironolactone administered in either single or divided doses is recommended, but may range from 25 to 200 mg daily. Combined therapy with other diuretics is indicated when more rapid diuresis is desired.
Primary hyperaldosteronism: After the diagnosis of hyperaldosteronism has been established, Spironolactone may be administered in doses of 100 to 400 mg daily in preparation for surgery. For patients who are considered unsuitable for surgery, Spironolactone may be employed for long-term maintenance therapy at the lowest effective dosage determined for the individual patient.
Essential hypertension: For adults, an initial daily dosage of 50 to 100 mg of Spironolactone administered in either single or divided doses is recommended.
Hypokalemia: Spironolactone in a dosage ranging from 25 mg to 100 mg daily is useful in treating diuretic-induced hypokalemia.
Interaction
ACE inhibitors: Taking ACE inhibitors and potassium-sparing diuretics at the same time has been linked to severe hyperkalemia.
Potentiation of orthostatic hypotension can occur when using alcohol, barbiturates, or opioids.
Corticosteroids, ACTH: Electrolyte depletion, particularly hypokalemia, may be exacerbated.
Lithium: Lithium should not be taken with diuretics in most cases. Lithium clearance is reduced by diuretics, which increases the risk of lithium poisoning.
Spironolactone has been found to prolong the half-life of digoxin.
Contraindications
Patients with acute renal insufficiency, substantial impairment of renal function, anuria, hyperkalemia, or sensitivity to Spironolactone should not use it.
Side Effects
Spironolactone use has been linked to the development of gynecomastia. GI symptoms such as cramps and diarrhea, tiredness, lethargy, headache, urticaria, mental disorientation, impotence, irregular menses or amenorrhoea, and postmenopausal hemorrhage are some of the other side effects.
Pregnancy & Lactation
During pregnancy, spironolactone should not be used.
Lactation: Canrenone, a Spironolactone active metabolite, is found in breast milk. If the medicine is deemed necessary, a different way of infant feeding should be implemented.
Precautions & Warnings
All diuretic-treated patients should be monitored for signs of fluid or electrolyte imbalance. Hyperkalemia can occur in persons with compromised renal function or who consume too much potassium, and it can lead to deadly heart abnormalities.
Storage Conditions
Store in a cold, dry, and light-protected location. Keep out of children's reach.
